practitioner and has not represented herself in that manner.
• Ruth-Ellen Danquah does not diagnose conditions, or illnesses, prescribe medication,
perform medical treatments and will not interfere with a licensed medical
professional.
• It is highly recommended that I see a medical professional for any physical or
psychological ailments I may have.
• Ruth-Ellen Danquah has made no promises to me as to the outcome of a Reiki
distance healing session.
• My decision to receive a Reiki distance healing from Ruth-Ellen Danquah is of my
own free will.
I, for myself, my heirs, successors, executors, administrators and assignees, hereby
release and forever discharge Ruth-Ellen Danquah, her heirs, successors, executors,
administrators, and assignees from any and all allegations, causes of actions, claims and
demands for any reason of any damage, loss or injury, to person and property which
heretofore has been or hereafter may be sustained in consequence of any treatment I may
receive, in any respect of and for any attempts by myself or anyone on my behalf to
cause temporary or permanent relief from the symptoms of any injury, ailment or disease
with which I have been diagnosed.
